１０日ほど前に、左足の踵の腫瘍切除のため、断脚手術を受けたたま様。

その足を丸ごと（？）バイオプシー（生検）に出しまして。
そのたま様の検査結果が出ました！
腫瘍は「 intermediate grade fibrosarcoma（中級 繊維肉腫）」だそうです。
ドクターからのメールによると
足のリンパ節への転移は見当たらない。
でもこのタイプの腫瘍は胸に広がりやすいのでレントゲン検査必須。
最初の１８ヶ月は３ヶ月ごとに。そのあとは半年ごとに。
ドクターがオンコロジー（腫瘍学）チームと話したところ
このタイプの腫瘍にはキモセラピー（抗がん剤）も考慮した方が良いかもしれないということ。
なぜなら、たま様のこの腫瘍は分裂速度が高速だから。
もしキモセラピーも考慮するなら、オンコロジーチームと話し合ってね
とのことでした。

英語が難しすぎて（そもそも日本語でも難しい）
Googleで検索しながら読み解きました。
繊維肉腫って、ワクチンやマイクロチップ打ったところからなったりするんだって。
たま様がいつもの病院でどこにワクチン打たれてたのかは知らないけれど、ワクチン打つのも考えものだなあと思いました🥲
そしてマイクロチップ！そういえば、たま様のマイクロチップはどこに埋め込まれてるんだろう！？
ハワイでは保護機関から里親になる際に、すでにマイクロチップは埋め込まれているのだ。
とりあえず今の所、転移してないっていうのは分かった！！！！
キモセラピー（抗がん剤）は乗り気じゃないのですが、とりあえず来週、抜糸の時にドクターに会えるので、そこでまた色々と聞いてきます。
みゃ！

MICROSCOPIC FINDINGS:
Haired skin and subcutis: Fibrosarcoma, intermediate-grade,
infiltrative
Lymph node: Lymphoid hyperplasia and plasma cell hyperplasia.
MARGINS:
Inked (green) tissue margins were free of neoplastic cells.
Mass to the closest soft tissue margin measured 7.5 cm.
Mass to the proximal bone margin measured 18.5 cm.
COMMENTS:
I favor this spindle cell sarcoma to be an intermediate grade
fibrosarcoma. I can not entirely rule out an intermediate grade
nerve sheath tumor but thought this diagnosis less likely. Regardless
of the terminology selected, all these tumor types are characterized
by frequent local invasion and recurrence but less frequent metastatic
disease. However, they can be quite destructive locally and
consequently I suggest close monitoring. Aggressive excision may
prevent recurrence. Regardless monitoring of the amputation site etc.
is advised. The tumor was graded according to a scheme proposed by
Dobromylskyj et al 2020. A lymph node was included and there was no
evidence of metastasis.
A new study (see reference) proposes a prognostic grading scheme based
on mitotic index, tumor necrosis, and inflammation. In this study,
cats with grade 1 (low-grade) tumors had a median survival time (MST)
of 900.5 days; with grade 2 (intermediate-grade) tumors, MST was 514
days; and with grade 3 (high-grade) tumors, MST was 283 days. Poor
outcome was most often due to local recurrence of the tumor, and
metastasis is uncommon to rare. Complete surgical excision is the
treatment of choice. Clinical staging is advisable.
MICROSCOPIC DESCRIPTION:
Sections of a poorly delineated, densely cellular sarcomatous mass in
connective tissue that consists of cells arranged in hapahzard sheets,
interlacing bundles, zigzag and storiform patterns with minimal
collagenous and/or myxoid stroma. These cells are spindle to fusiform
with plump pleomorphic nuclei and a moderate mitotic rate of 21
mitotic figures per 10 high powered fields. There is infiltration of
the immediate stroma with extension into the overlying dermis
affecting ulceration, but no obvious invasion of vessels. There was
extensive tumor necrosis and hemorrahage with scattered mixed
inflammation. Underlying bone was not involved.
A lymph node was included and had lymphoid hyperplasia and plasma cell
hyperplasia but metastatic neoplasia was not identified.
